Group Adult Foster Care (GAFC)

What is Group Adult Foster Care?

This Medicaid-sponsored program provides daily personal care and homemaking assistance to disabled people 22 and older that lives in approved subsidized housing in our five-town service area.

People served by this program are also at risk of nursing home placement due to functional or cognitive impairment.

Do you qualify for Group Adult Foster Care?

You may be eligible if you have:

  • MassHealth
  • CommonHealth
  • Massachusetts Commission for the Blind
  • Because of a chronic or permanent doctor-certified disability or illness, you need daily assistance with personal care to live safely at home.
  • Your doctor prescribes personal assistant services for you.
  • You reside in approved subsidized housing.
  • You have been approved by the Massachusetts Department of Medical Assistance.
  • You are 22 years of age or older.

Our Group Adult Foster Care program can simplify your life by providing someone to help you with:

  • Getting dressed
  • Bathing
  • Moving in and out of bed
  • Getting around
  • Using the toilet
  • Skincare
  • Medication reminders
  • Meal preparation
  • Housekeeping
  • Laundry
  • Shopping
  • Transportation (rides door-to-door, to medical appointments for people 60 and older; or to anywhere for anyone with a disability)
